Eligibility for Self-Advocacy Services

For Students (Ages 14-22)

If you are a student between the ages of 14 and 22, have been diagnosed with a disability, you are eligible to receive self-advocacy services. This service is available through Adult Career and Continuing Education Services - Vocational Rehabilitation (ACCES-VR).

Students who are between the ages of 14-22 and not yet connected with ACCES-VR are eligible to receive this service as a Potentially Eligible (PE) participant.

Steps to Get Started:

1. Complete the Referral Form:

   • For individuals under 18, a parent or legal guardian must complete the referral form. 

2. Download the Form:

   • You can download the form and open it with Adobe Reader (free).

3. Fill Out the Form:

   • Ensure all required information is complete.

4. Sign the Form:

   • Print the form and sign where indicated - (ACCES-VR requires a wet signature) 

5. Return the Completed Form:

   • Once completed, return the form to Assistive Technology Solutions and Services for processing.

Vocational Rehabilitation Service Eligibility

If you are between 14-22 years old and already connected with ACCES-VR and would still like to receive self-advocacy services, you can request this service through ACCES-VR.

If you are an adult who would like to receive services through ACCES-VR, please follow the process below. 

1. Visit the ACCES-VR Website:

   • Head to the ACCES-VR site for instructions on how to apply and to complete the ACCES-VR application form.
