If you are a student between the ages of 14 and 22, have been diagnosed with a disability, you are eligible to receive self-advocacy services. This service is available through Adult Career and Continuing Education Services - Vocational Rehabilitation (ACCES-VR).
Students who are between the ages of 14-22 and not yet connected with ACCES-VR are eligible to receive this service as a Potentially Eligible (PE) participant.

Complete the Referral Form:
For individuals under 18, a parent or legal guardian must complete the referral form.

Ensure all required information is complete.
Sign the Form:
Print the form and sign where indicated - (ACCES-VR requires a wet signature)
Return the Completed Form:
Once completed, return the form to Assistive Technology Solutions and Services for processing.
